Care Coordination Coach
The primary function of the Care Coordination Coach is to work collaboratively with the Director, Senior Living Initiatives in the development of the senior living community (SLC) program. This individual will develop strong partnerships with SLC staff, Visiting Nurse, Hospice Atlanta and Care Management Services staff as well as with home health and hospice clinical teams and managers. This individual will provide direct support and services to identified communities in his/her area and will assist in the ongoing development of existing customers as well as the acquisition of additional communities.
EDUCATIONAL REQUIREMENTS: Bachelors or Associates Degree in Nursing or Graduate of an accredited Licensed Practical Nursing Program.
LICENSURE: Clinical Licensure in the State of Georgia; Must have valid and current Drivers license and auto insurance.
EXPERIENCE REQUIREMENTS: 2-3 years previous work experience in senior living facilities preferred, 3 to 5 years of combined clinical and administrative/marketing experience, preferably with some experience in hospital or community based home health agency.
JOB KNOWLEDGE: Knowledge of the Atlanta health care market, a plus; knowledge of senior living facilities, clinical and administrative experience.
SKILLS: Proven interpersonal skills and effective communication skills, excellent customer service and problem solving skills. Experience and comfort with teaching and/or speaking in large group presentations.
COMPUTER SKILLS: Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
TRANSPORTATION: Must have reliable transportation.
